Before your surgery, you can print out the following forms to
read and fill-out.
- Patient Health Assessment Questionnaire
Please fill this form out a week before surgery and send it to:
Culpeper Surgery Center, 541 Sunset Lane, Suite 201, Culpeper
VA 22701, or fax: 540-829-8191.
- Privacy Notice
You can bring the signed Privacy Notice the day of surgery.
The Culpeper Surgery Center is located next to Culpeper Regional Hospital off of Sunset Lane.
Once you have registered with the receptionist we will begin the pre-op check in process.
Please arrive at least 1 hour before your scheduled surgery time and 45 minutes before your endoscopy.
Check in at the appointed time. You will be prepared for surgery,
medically assessed and our anesthesiologist will speak with you regarding
anesthesia options and answer your questions.
One family member or friend should remain in the waiting room
during your surgery, in case of emergency. A legal guardian must be with
children under 18 years of age. Please leave all
valuables at home or in the car. Cell phones interfere with
medical monitors and thus may be used only outside of the building. A courtesy
phone is available for your use at our
surgery center.
